This programme is a structured disease management initiative for members diagnosed with Type 1 or Type 2 diabetes. It provides access to coordinated, clinically guided care that helps reduce complications and improve long-term health outcomes.
The programme is available at no additional cost to members and is managed in collaboration with healthcare professionals, including GPs, endocrinologists, dietitians, and diabetes educators.

By enrolling in the Diabetes Programme, members can access:
- Regular Consultations: Covered GP and specialist visits to monitor and manage your diabetes, including glucose level tracking and treatment adjustments.
- Prescribed Medication: Access to approved chronic medication according to the scheme’s formulary or list.
- Blood Tests & Screenings: Routine pathology tests, kidney function, and lipid profile checks.
- Diabetes Education: Support from trained educators to understand your condition and manage your lifestyle more effectively.
- Dietary Support: Nutritional advice and meal planning sessions with registered dietitians.
- Podiatrist Visit: Annual foot screening and care guidance.
- Eye Screening: Annual eye exam to detect early signs of diabetic eye disease.
- Digital Health Tools: Access to health tools such as no-prick’n-bleed and virtual consultations for easy tracking and remote care.
- Care Coordination: Seamless referrals and follow-ups across your healthcare team.
Benefits Allocation
All benefits are funded from the Risk Benefit (not from your day-to-day or out-of-hospital limits).
Who Qualifies
The programme is available to members who have been diagnosed with diabetes.
- Automatic Identification: Members may be identified for registration based on claims data (e.g., for diabetes medication, consultations, or hospital admissions).
- Voluntary Registration: Members and their healthcare providers may also request registration by contacting SizweHosmed.
