Disease Risk Management (DRM) Programme

This programme aims to support members living with chronic conditions by offering health education, clinical guidance, and access to authorised medical services while promoting adherence to prescribed treatments.

Programme Benefits

  •  Members registered on the DRM Programme who have authorised chronic medication will receive a tailored Care Plan.
  • This plan includes approved consultations, blood tests, and radiological tests related to managing your condition.

Telephonic follow-ups by healthcare professionals to:

  • Provide health education and improve understanding of your condition.
  • Assist in getting authorisations for necessary medical services.
  • Coordinate treatment with your treating doctor.

Who Qualifies

Automatic Identification

Members may be identified for registration through claims data (e.g. chronic medication, consultations, or hospital admissions).

Voluntary Registration

Members may also contact the DRM Programme directly following diagnosis with a chronic condition to request registration.

How to Register a Chronic Condition

  • Have your provider complete the Chronic Application Form. Click here to download.
  • Have a copy of your prescription showing the ICD-10 code.
  • Have the relevant pathology results based on the condition you are applying for.
  • You may contact the Pharmacy Benefit Management department should you have any queries, on 0860103455.
