- Ongoing Support
Telephonic follow-ups by healthcare professionals to:
- Provide health education and improve understanding of your condition.
- Assist in getting authorisations for necessary medical services.
- Coordinate treatment with your treating doctor.
Who Qualifies
Automatic Identification
Members may be identified for registration through claims data (e.g. chronic medication, consultations, or hospital admissions).
Voluntary Registration
Members may also contact the DRM Programme directly following diagnosis with a chronic condition to request registration.

How to Register a Chronic Condition
- Have your provider complete the Chronic Application Form. Click here to download.
- Have a copy of your prescription showing the ICD-10 code.
- Have the relevant pathology results based on the condition you are applying for.
